Interesting, we just went to Ceiba last weekend for a coffee and a sweet pastry - quite nice actually. But they don't mention the relatively new Restaurante Botin in the Old San Juan, maybe because it just isn't too impressive? O ordered octopus if I am not mistaken ;-) and he sent it back because it was too dry. In general we avoid Spanish restaurants, it feels silly to pay lots of money for plates of chorizo and manchego cheese which we can find in Costco, or tortilla which O makes so much better at home!
